PRESCRIPTIONS If your prescription does not have a refill, it is probably time for you to follow-up in the office to re-assess your medical condition. Regular check-ups are important to maintain your health. Our providers may feel uncomfortable refilling prescriptions without regular follow-up visits. In general, we follow this recommended schedule of follow-up visits: Diabetes—every 3 months. Hypertension—every 6 months. Asthma—every 6 months. Mental illness—every 6 months. General health—yearly.
We try very diligently to maintain accurate records. Please bring a complete list of ALL of your medications to EVERY office visit.
Usually we fill your regular medications at your annual examination in 3 month supplies and 4 refills so that mid-year renewals are not required. If prescription renewals are needed, please contact your pharmacy; generally they will fax us, and this tends to speed the process....typically 1 to 3 days. If the prescription is mail order and requires a written prescription, it may take 3-4 business days to be processed. A fee of $5 will be assessed for any prescriptions (particularly controlled substances) written without an appointment.
TESTS For blood-draw services we normally use CPL (Clinical Pathology Laboratory). If you or your insurance company prefers another lab, it is your responsibility to inform a medical staff member before the specimen is taken. This prevents being billed for lab work processed by CPL, instead of another preferred lab. Labs, X-ray reports, and other test results need to be reviewed by your health care provider; this may take 3-4 business days. A staff member will contact you sooner if the results are urgent; otherwise you will be contacted by phone or mail with the results. Please let us know if you would prefer email correspondence. We will contact you for every test result, normal or otherwise; if you do not hear from us within a week, please let us know so that we may track it down. With our electronic medical record system, your labs are now available online. Ask your provider how to set this up at your next office visit.
REFERRALS Please allow several days for routine referrals to be processed through your insurance. You should be aware if your insurance requires a referral or authorization to a specialist, or for any Imaging (MRI, CT, etc.). It is your responsibility to make sure the referral or authorization was obtained before any procedures or office visits that may require them. ADDITIONAL SERVICES A fee of $10 will be assessed for any paperwork or forms needing completion without an appointment. Depending on the complexity of the form, it may take up to 10 days to be completed.
*There will be a fee of $25 for missed appointments or appointments not cancelled within 24 hours ($50 for Annual Wellness Visits). Please notify us as soon as possible if you cannot make your scheduled appointment.
