Proof of Insurance: Please be prepared to provide your current insurance card at every office visit in addition to your drivers license so that we can copy them for your chart. It is your responsibility to notify us of any changes in your insurance coverage so that SOFM can help you receive your maximum benefits. If you fail to provide us with the correct insurance information in a timely manner, you may be responsible for the balance of a claim.
Payment Policy: Copays are due at the check in time of your appointment and are not billable. We will also collect deductibles and any previous balances on your account. Filing insurance claims is a courtesy and we will gladly do this for our patients.If you are insured with a plan we are not contracted with or do not have medical insurance, the patient is required to pay in full at the time of service. For your convenience we accept Cash, Check, Visa and MasterCard cards for payment. You may also remit payment for balances on your account at our patient portal payment site: https://pay.instamed.com/southoaks.fm
Non-covered Services: Please be aware that some of the services you receive may be noncovered or not considered reasonable or necessary by your medical insurance company. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have reguarding your coverage, ie: immunization limitations, lesion removal and biopsy coverage, trigger point injections, or how often you may have a physical exam...
Secondary Insurance: We do not bill your secondary insurance for copays. Copays are collected at the time of service. Should you have a deductible to meet with your primary insurance we will file a claim to your secondary insurance if SOFM is a contracted provider.
Third Party Payers: Our office does not bill third party payers such as Personal Injuries Protection for a motor vehicle accident, or attorneys.
Workmen’s Compensation:We do not participate in this insurance plan or manage on the job injuries. However, we can see you for the initial visit.
Returned Check: There is a returned check fee of $35.00 should your payment not clear the bank. *Extended family members bringing children in must pay copays/deductibles/balances just as if the parent brought them in. In cases of divorced parents, the parent bringing the child to the visit will be deemed responsible for payment. Our office does not become involved in custody disputes over which parent is the responsible billing party.
