¨ There may be a fee of $25 for missed appointments or appointments not cancelled within 24 hours. Please notify us as soon as possible if you cannot make your scheduled appointment.
¨ Due to current HIPAA regulations, we are unable to give HIV testing results out over the phone or by mail/e-mail. You must come in to our office to get these results in person. You do not need an appointment and these are generally available within 1-2 days depending on your lab.
¨ A fee of $10 may be assessed for any paperwork or forms to be completed without an appointment and that it may take up to 10 days to be completed.
¨ Prescription renewals are to be processed through the requested pharmacy. If the prescription is mail order and requires a written prescription, it may take 3-4 business days to be processed.
¨ A fee of $5 may be assessed for any prescriptions (including triplicates) written without an appointment.
¨ If your prescription does not have a refill, it is probably time for you to follow-up to re-assess your medical condition. Regular check-ups are important to maintain your health. Our providers may feel uncomfortable refilling prescriptions without regular follow-up visits. In general, we follow the recommended schedule of follow-up visits: Diabetes—every 3 months. Hypertension—every 6 months. Asthma—every 6 months. Mental illness—every 6 months. General health—yearly.
¨ For blood-draw services we normally use CPL (Clinical Pathologies Laboratory). If you or your insurance company prefers another lab, it is your responsibility to inform a medical staff member before the specimen is taken to prevent being billed for having lab work processed by CPL, instead of another preferred lab.
¨ Labs, x-ray reports, and other test results need to be reviewed and it may take 3-4 business days to be reviewed. A staff member will contact you sooner if the results are urgent, otherwise you will be contacted by phone or mail with the results. However, we call on all test results, normal or otherwise. If you do not hear from us within a week, please let us know so that we may track these down.
¨ Please allow several days for routine referrals to be processed through your insurance. You should be aware if your insurance requires a referral or authorization for any Imaging (MRI, CT, etc.) or to a specialist (Dermatologist, Cardiologist, Surgeon, etc.) It is your responsibility to make sure the referral or authorization was obtained before any procedures or office visits that may require them. This way, you can avoid any out-of-pocket or out-of-network expenses that may be incurred. |
